| 目的 分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清白细胞介素(IL)-6、IL-10、25-羟维生素D [25(OH)D]、钙(Ca)、磷(P)水平与继发骨质疏松症的关系。方法 选择我院2021年1月至2023年12月诊治的180例T2DM患者,利用双能X光骨密度仪测定骨密度,并以T值表示,根据T值将患者分为骨量正常组、骨量减少组、骨质疏松组,测量各组患者的血清IL-6、IL-10、25(OH)D、Ca、P水平,分析血清IL-6、IL-10、25(OH)D、Ca、P与继发骨质疏松症的关系,并在诊断T2DM患者是否存在继发骨质疏松症的效能。结果 不同组别患者的血清IL-6、IL-10、25(OH)D、Ca、P水平比较有明显差异(P<0.05);血清IL-6、IL-10、25(OH)D、Ca、P和T2DM患者继发骨质疏松症存在相关性(P<0.05);ROC曲线分析发现,在评估T2DM患者是否继发骨质疏松症时,IL-6的AUC、敏感度、特异度分别为0.805、79.03%、68.52%,IL-10则为0.854、82.26%、77.78%,25(OH)D为0.892、82.26%、87.04%,Ca为0.828、80.65%、72.22%,P为0.939、90.32%、87.04%,联合则为0.974、91.94%、92.59%,联合诊断的AUC、敏感度、特异度相对较高。结论 T2DM患者血清IL-6、IL-10、25(OH)D、Ca、P和继发骨质疏松症存在一定关系,能通过观察其水平变化,预测患者继发骨质疏松症的风险,以便给予患者合理有效的干预措施,改善其预后。 |

| Objective To analyze the relationship between serum levels of interleukin-6, IL-10, 25 vitamin D[25(OH)D], calcium (Ca), phosphorus (P) and secondary osteoporosis in patients with type 2 diabetes mellitus (T2DM). Methods A total of 180 T2DM patients diagnosed and treated in the hospital from 2021.01 to 2023.12 were selected. BMD was measured by dual-energy X-ray bone density analyzer, and was expressed by T value. According to T value, the patients were divided into normal bone mass group, osteopenia group and osteoporosis group, and serum levels of IL-6, IL-10, 25(OH)D, Ca and P were measured in each group. To analyze the relationship between serum IL-6, IL-10, 25(OH)D, Ca, P and secondary osteoporosis, and to evaluate the diagnostic efficacy of secondary osteoporosis in T2DM patients. Results There were significant differences in serum levels of IL-6, IL-10, 25(OH)D, Ca and P among different groups (P<0.05). Serum IL-6, IL-10, 25(OH)D, Ca, P were correlated with secondary osteoporosis in T2DM patients (P<0.05). ROC curve analysis showed that the AUC, sensitivity and specificity of IL-6 were 0.805, 79.03% and 68.52%, and that of IL-10 were 0.854, 82.26% and 77.78%, respectively, in evaluating whether T2DM patients had secondary osteoporosis. 25(OH)D was 0.892, 82.26%, 87.04%, Ca was 0.828, 80.65%, 72.22%, P was 0.939, 90.32%, 87.04%, and combined was 0.974, 91.94%, 92.59%. The AUC, sensitivity and specificity of combined diagnosis were relatively high. Conclusion There is a certain relationship between serum IL-6, IL-10, 25 (OH) D, Ca, P and secondary osteoporosis in T2DM patients. By observing the changes in their levels, the risk of secondary osteoporosis in patients can be predicted, so as to provide reasonable and effective intervention measures and improve their prognosis. |
