      骨质疏松症(osteoporosis,OP)是一种因骨代谢失衡导致骨微结构破坏,骨脆性增加,易发生骨折的临床疾病。研究显示我国OP患者数量众多,且随着人口老龄化趋势的加剧,患者数量将持续增长。OP已成为我国严重的公共卫生问题,对中老年人群的健康构成了严重威胁。目前OP的治疗主要以抗骨吸收药物和促进骨合成的药物为主,但长期使用后,不仅疗效会减弱,还易发生严重不良反应,而且OP需持续乃至终身治疗,因此探索一种安全、经济的治疗方法尤为迫切。近年来,表观遗传修饰在各个领域中的研究得到广泛关注,其中N6-甲基腺苷 (N6-methyladenosine,m6A)甲基化修饰作为真核生物中最丰富的RNA修饰形式成为新的研究热点。研究表明,部分中药通过作用于m6A修饰途径,促进了机体细胞的成骨分化,从而加速骨骼的形成与修复,这对于防治OP及降低骨折风险有着重要意义。因此,本文综述了m6A甲基化修饰在调控成骨细胞(osteoblast,OB)和破骨细胞(osteoclast,OC)分化中的机制,探讨了中医药治疗OP的潜力,并总结了与m6A甲基化修饰相关的研究进展,旨在将中医药自身优势与m6A甲基化理论相结合,希冀从m6A甲基化的角度为今后临床中医药防治OP提供一些治疗参考。

      Osteoporosis is a common clinical bone disease that causes bone microstructure destruction, increased bone fragility, and prone to fractures due to the imbalance of bone metabolism. Epidemiological studies show that the number of OP patients in China is huge. The number of patients will continue to increase with the intensification of the aging trend of the population. OP has become a serious public health problem in China, posing a serious threat to the health of middle-aged and elderly people. At present, the clinical treatment of OP is mainly based on anti-reabsorption drugs and drugs that promote bone synthesis. However, after long-term use, these drugs will not only weaken their efficacy, but also be prone to serious adverse reactions. OP treatment usually needs to be continued or even lifelong, so it is particularly urgent to explore an economical and safe treatment. In recent years, the research of epigenetic modification in various fields has received extensive attention. N6-methyladenosine (m6A) methylation modification has become a new research hotspot as the most abundant RNA modification mode is in eukaryotes. Studies have shown that some traditional Chinese medicines may effectively promote the osteogenic differentiation of body cells by targeting the m6A modification pathway, thereby accelerating the formation and repair of bones, which is of great significance for the prevention and treatment of OP and for the reduction of fracture risk. Therefore, this article summarizes the mechanism of m6A methylation modification in regulating the differentiation of OB and OC, discusses the potential of traditional Chinese medicine in the treatment of OP, and summarizes the research progress related to m6A methylation modification, aiming to combine the advantages of traditional Chinese medicine with the theory of m6A methylation, hoping to provide some therapeutic references for the prevention and treatment of OP in clinical Chinese medicine in the future from the perspective of m6A methylation.
