      骨质疏松症是由于骨骼的退行性改变,影响骨重建失衡导致的全身性骨代谢疾病,表现为骨量减少、骨脆性增加及骨折风险升高,并在严重威胁着老年人群健康。虽然现有新兴药物如双膦酸盐、地舒单抗等可抑制骨吸收从而减缓骨肌系统的退变,但长期应用导致依从性差、不良反应明显且疗效不显著,因此亟需新型干预手段以抗骨质疏松。OPG/RANK/RANKL信号通路是调控骨重建及骨代谢的关键信号通路,其中RANKL结合RANK可激活破骨细胞,OPG则通过竞争结合发挥拮抗作用。近年来研究发现,该通路不仅受骨细胞直接调控,还与神经递质、内分泌激素及免疫细胞因子密切相关,形成以“神经-内分泌-免疫(Neuro-Endocrine-Immune, NEI)网络”为基础的多维调控体系。中药具有多成分、多靶点的优势,可通过上调OPG、抑制RANKL及阻断信号转导实现骨质疏松症的综合干预。其中淫羊藿苷、补骨脂素、丹参素等活性成分,以及左归丸、仙灵骨葆等复方,均可在不同层面调控NEI网络与OPG/RANK/RANKL信号互作,从而兼具补肾填精、抗炎免疫、神经内分泌调节等作用。本文在总结中药调控该通路研究的基础上,重点从NEI网络视角加以整合,为骨质疏松的防治及中医药精准应用提供新思路。

      Osteoporosis is a systemic metabolic disorder marked by reduced bone mass, increased fragility, and fracture risk, posing a major health burden in aging populations. Current therapies such as bisphosphonates and denosumab suppress bone resorption but show limited efficacy, poor compliance, and notable side effects with long-term use, highlighting the need for novel interventions. The OPG/RANK/RANKL pathway plays a central role in bone remodeling, where RANKL binding to RANK activates osteoclasts and OPG acts as a decoy receptor. Recent studies show this pathway is also influenced by neurotransmitters, endocrine hormones, and immune cytokines, forming a neuro–endocrine–immune (NEI) regulatory network. Traditional Chinese medicine (TCM), with multi-component and multi-target advantages, can upregulate OPG, inhibit RANKL, and block downstream signaling to improve bone metabolism. Compounds such as icariin, psoralen, and tanshinone, and formulas including Zuogui Pill and Xianling Gubao, modulate both the NEI network and OPG/RANK/RANKL signaling, exerting kidney-tonifying, anti-inflammatory, and neuroendocrine-regulating effects. This review highlights advances in TCM regulation of this pathway, offering new perspectives for osteoporosis prevention, treatment, and precision application.
