      原发性骨质疏松症(primary osteoporosis,POP)是一种以骨密度降低和局部骨微结构破坏为主要临床特征的慢性代谢性骨病。从中医理论来看,骨质疏松症的发生与脾胃、肾精等脏腑功能失调密切相关。本文深入探讨金元时期医家李东垣在《脾胃论》中提出的“火与元气不两立”理论,并基于此理论分析原发性骨质疏松症的病因病机及临床论治。认为脾胃虚弱、骨肉失养为发病基础,阴火鸱张、热灼骨干为中心环节,肾精亏虚、骨骼枯槁为终末转归。基于上述病因病机,治疗应注重调理脾胃、清降阴火、滋养肾精,以恢复脏腑功能的平衡。因此,本文提出建中焦、伏阴火、滋肾精的治疗原则,旨在为骨质疏松症的临床治疗提供新的思路。

      Primary osteoporosis (POP) is a chronic metabolic bone disease characterized by decreased bone mineral density and local bone microstructure destruction. From the theory of traditional Chinese medicine, the occurrence of osteoporosis is closely related to the dysfunction of spleen and stomach, kidney essence and other organs. In this paper, the theory of 'fire and primordial qi are incompatible' proposed by Li Dongyuan, a doctor in the Jin and Yuan Dynasties, in the ' theory of spleen and stomach ', and based on this theory, the etiology, pathogenesis and clinical treatment of POP are analyzed. The article believes that the weakness of the spleen and stomach and the loss of bones and muscles are the basis of the disease. Yin fire and burning the backbone are the central links. The deficiency of kidney essence and the withered bones are the final outcome. Based on the above etiology and pathogenesis, the treatment should focus on regulating the spleen and stomach, clearing and lowering yin fire, nourishing kidney essence, so as to restore the balance of viscera function. Therefore, this paper puts forward the treatment principles of Jianzhongjiao, Fuyinhuo and Zishenjing, aiming to provide new ideas for the clinical treatment of osteoporosis.
