| 绝经后骨质疏松症（postmenopausal osteoporosis，PMOP）作为涉及多系统、多途径致病的慢性复杂性骨病，严重危害中老年女性健康。中医药防治PMOP具有疗效确切、副作用小、多靶点调控等优势，已形成较为系统的理论体系。基于中医“肾主骨生髓”理论框架，结合现代临床实践发现，PMOP的病因病机除肾精亏虚外，还涉及肝失疏泄、脾胃虚弱、血瘀痰阻等多因素交互作用，这一发病机制已体现在《中医药防治原发性骨质疏松症专家共识》等多项权威指南中。本研究通过系统梳理国内外文献，重点解析经典方剂、经验方及中成药治疗PMOP的中医辨证规律（涵盖肝肾同补、健脾活血、祛痰通络等治则）及其分子作用机制，旨在构建中医整体观指导下的病机演变模型，促进“辨证-立法-拟方”诊疗模式与现代医学理论的深度整合，为优化PMOP个体化防治方案提供循证依据。 |

| Postmenopausal osteoporosis (PMOP), as a chronic and complex bone disease involving multiple systems and pathways, seriously endangers the health of middle-aged and elderly women. Traditional Chinese medicine (TCM) has advantages in the prevention and treatment of PMOP, such as definite therapeutic effects, few side effects, and multi-target regulation, and has formed a relatively systematic theoretical system. Based on the TCM theory framework of "kidney governing bone and marrow production", combined with modern clinical practice, it is found that the etiology and pathogenesis of PMOP, in addition to kidney essence deficiency, also involve the interaction of multiple factors such as liver qi stagnation, spleen and stomach weakness, blood stasis and phlegm obstruction. This pathogenesis has been reflected in multiple authoritative guidelines such as the "Expert Consensus on the Prevention and Treatment of Primary Osteoporosis with Traditional Chinese Medicine". This study systematically reviews domestic and foreign literature, focusing on analyzing the TCM syndrome differentiation rules (including liver-kidney tonification, spleen-strengthening and blood-activating, phlegm-resolving and meridian-unblocking, etc.) and molecular mechanisms of action of classic prescriptions, empirical prescriptions and Chinese patent medicines in the treatment of PMOP, aiming to construct a pathogenesis evolution model guided by the TCM holistic view, promote the in-depth integration of the "syndrome differentiation - legislation - prescription formulation" diagnosis and treatment model with modern medical theory, and provide evidence-based basis for optimizing individualized prevention and treatment plans for PMOP. |
