Osteoporosis (OP) is a metabolic bone disease characterized by decreased bone mineral density and abnormal bone microstructure, with a significantly increased risk of fragility fractures. Bromodomain-containing protein 4 (BRD4), an important member of the bromodomain and extra-terminal domain proteins (BET family), is an epigenetic regulatory factor that plays a key role in stem cell differentiation, cell cycle, inflammatory response, and cancer development. Recent studies have shown that BRD4 plays an important role in the pathogenesis of OP. BRD4 promotes bone formation by regulating the WNT4/NF-κB, Wnt/β-catenin, and Runx2 pathways, and by inhibiting bone resorption through the RANKL/NF-κB/MAPK signaling pathway, exerting a bidirectional regulatory effect in the progression of OP. Therefore, BRD4 may become a new target for the treatment of OP and provide a new direction for the prevention and treatment of OP. This article summarizes the mechanism of BRD4 in OP and explores the potential targets and possible treatment approaches of BRD4 in OP. Future research should explore the specific mechanism of BRD4 in OP and develop new therapeutic drugs to improve the clinical treatment effect of OP.
