      目的 探讨绝经后骨质疏松患者血清β-链蛋白(β-catenin)、基质金属蛋白酶-9(MMP-9)及白介素-10(IL-10)水平与骨代谢的关系。方法 选取南京浦口人民医院2022年8月-2024年5月收治的92例绝经后骨质疏松患者作为骨质疏松组,另选择同期本院体检的92例绝经后健康女性作为对照组。检测并分析两组的血清β-catenin、MMP-9、IL-10、N-骨钙素(N-BGP)、Ⅰ型胶原氨基端前肽(PINP)、β-Ⅰ型胶原交联C端肽(β-CTX)的水平变化;分析其与骨质疏松的相关性。结果 骨质疏松组血清β-catenin、IL-10水平低于对照组,MMP-9、N-BGP、PINP、β-CTX高于对照组(P<0.05);血清β-catenin、IL-10与骨代谢指标N-BGP、PINP、β-CTX呈负相关,血清MMP-9与骨代谢指标N-BGP、PINP、β-CTX呈正相关(P<0.05);β-catenin和IL-10是绝经后骨质疏松发生的保护因素,MMP-9、N-BGP、PINP以及β-CTX指标为绝经后骨质疏松发生的危险因素(P<0.05);ROC曲线分析显示,血清β-catenin、MMP-9、IL-10预测绝经后骨质疏松的AUC分别为分别为0.874、0.891、0.860,截断值分别为22.144(pg/mL)、52.144(ng/mL)、30.778(μg/L),敏感性分别为72.83 %、69.57 %、88.04 %,特异性分别为86.96 %、96.74 %、69.57 %;三者联合预测的AUC为0.982,截断值为0.421,敏感性为95.65 %,特异性为94.57 %。结论 绝经后骨质疏松患者血清β-catenin、IL-10水平都明显降低,MMP-9明显升高,且均为绝经后骨质疏松的影响因素,三者与骨代谢指标密切相关,均可作为预测绝经后骨质疏松的指标,且三者联合预测价值更高。

      Objective To explore the relationship between serum levels of β-catenin, matrix metalloproteinase-9 (MMP-9), and interleukin-10 (IL-10) and bone metabolism in menopausal osteoporosis patients. Methods A total of 92 postmenopausal osteoporosis patients admitted to Nanjing Pukou People’s Hospital from August 2022 to May 2024 were selected as the osteoporosis group, and another 92 postmenopausal healthy women who underwent physical examination at the same hospital during the same period were selected as the control group. The levels of serum β-catenin, MMP-9, IL-10, N-terminal bone gamma-carboxyglutamate (N-BGP), type I collagen amino-terminal propeptide (PINP), and β-type I collagen cross-linked C-terminal peptide (β-CTX) were measured and analyzed in both groups. The correlation between these markers and osteoporosis was also analyzed. Results The serum levels of β-catenin and IL-10 in the osteoporosis group were lower than those in the control group, while the levels of MMP-9, N-BGP, PINP, and β-CTX were higher in the osteoporosis group than in the control group (P<0.05). Serum β-catenin and IL-10 were negatively correlated with bone metabolism markers N-BGP, PINP, and β-CTX, while serum MMP-9 was positively correlated with these bone metabolism markers (P<0.05). β-catenin and IL-10 were protective factors for postmenopausal osteoporosis, while MMP-9, N-BGP, PINP, and β-CTX were risk factors for the occurrence of postmenopausal osteoporosis (P<0.05). ROC curve analysis showed that the AUCs for serum β-catenin, MMP-9, and IL-10 in predicting postmenopausal osteoporosis were 0.874, 0.891, and 0.860, respectively, with cutoff values of 22.144 (pg/mL), 52.144 (ng/mL), and 30.778 (μg/L), and sensitivities of 72.83 %, 69.57 %, and 88.04 %, and specificities of 86.96 %, 96.74 %, and 69.57 %, respectively. The AUC for the combined prediction of the three markers was 0.982, with a cutoff value of 0.421, a sensitivity of 95.65 %, and a specificity of 94.57 %. Conclusion In postmenopausal osteoporotic patients, serum β-catenin and IL-10 levels are significantly reduced, while MMP-9 levels are significantly increased. All of these are influencing factors of postmenopausal osteoporosis. The three markers are closely related to bone metabolism indicators and can serve as predictors for postmenopausal osteoporosis. The combined prediction value of the three markers is higher.
