| 乳糜泻（celiac disease，CeD）是一种因摄入麸质而引发的免疫介导性肠道疾病。骨质疏松是该病常见的肠外并发症，主要表现为骨量减少和骨微结构破坏，并可导致骨骼脆性增加和骨折风险上升。其发病机制尚未完全阐明，目前认为与营养吸收障碍、遗传、免疫炎症因素、内分泌功能紊乱以及肠道菌群失调等多重环节有关。治疗上，严格的无麸质饮食是基础，配合健康生活方式有助于骨骼健康恢复，其他干预还包括药物疗法及肠道菌群调节等。本文综述了CeD伴骨质疏松的发病机制与治疗策略的最新研究进展，以期为临床诊治与管理提供参考。 |

| Celiac disease (CeD) is an immune-mediated enteropathy triggered by gluten ingestion. Osteoporosis is a common extra-intestinal complication of CeD, primarily manifested as reduced bone mass and deterioration of bone microarchitecture, leading to an increased bone fragility and a heightened risk of fractures. The pathogenesis has not been fully elucidated and is currently believed to involve multiple factors, including malabsorption and nutritional deficiencies, genetic and immune-inflammatory factors, endocrine dysfunction, and intestinal dysbiosis. Regarding to the treatment, a strict gluten-free diet (GFD) forms the cornerstone, complemented by a healthy lifestyle to support bone health restoration. Additional interventions include pharmacotherapy and modulation of the gut microbiota. This review summarizes recent advances in the pathogenesis and therapeutic strategies for osteoporosis-associated CeD, aiming to provide a reference for clinical diagnosis and management. |
