| 绝经后骨质疏松症（postmenopausal osteoporosis，PMOP）是一种与脂代谢紊乱密切相关的全身性疾病，其发病机制涉及骨髓脂肪异常堆积、成骨-成脂分化失衡等。中医药治疗PMOP具有多靶点调节脂-骨代谢的优势。本文从脂-骨代谢交互作用机制入手，结合中医“肾精亏虚-脾失健运-血瘀阻络”的病机理论，归纳了中医药通过铁死亡通路、PPAR信号通路、脂肪源性外泌体、Wnt/β-catenin通路等调节脂-骨代谢的机制，及淫羊藿、骨碎补等单味中药和补肾活血方、二仙汤等复方在改善骨微结构方面的作用。未来研究可借助多组学技术，进一步探讨中医药治疗PMOP的机制，为其防治提供新思路。 |

| Postmenopausal osteoporosis (PMOP) is a systemic bone metabolic disorder closely associated with lipid metabolism disorders. Its pathogenesis involves abnormal bone marrow fat accumulation and imbalance in osteogenic-adipogenic differentiation. Traditional Chinese medicine (TCM) offers advantages in treating PMOP through multi-target regulation of lipid-bone metabolism. This paper explores the mechanisms of lipid-bone metabolic interactions, integrating TCM theories of "kidney essence deficiency -Spleen Dysfunction-Blood Stasis Obstructing Collaterals" theory, summarizes TCM mechanisms for regulating bone-lipid metabolism through ferroptosis pathways, PPAR signaling pathways, adipogenic exosomes, and Wnt/β-catenin pathways. It also highlights the effects of single herbs like Epimedium and Dipsacus as well as formulas such as Kidney-Tonifying and Blood-Activating Decoction and Erxian Decoction in improving bone microarchitecture. Future research may leverage multi-omics technologies to further explore the mechanisms underlying TCM treatment for PMOP, offering novel insights for its prevention and management. |
