      目的:探索中老年高血压患者使用培哚普利治疗3年对骨密度及骨代谢指标的影响。方法:选择2013年9月~2015年7月我院老年高血压并骨质疏松患者138例为研究对象,随机、双盲分为治疗组(n=69)和对照组(n=69)。治疗组患者给予培哚普利治疗,对照组给予苯磺酸左旋氯氨地平片治疗。治疗3年,测定治疗前及治疗后36个月的血清I型胶原交联C 末端肽(CTX),抗酒石酸酸性磷酸酶(TRAP)和骨特异性碱性磷酸酶(BALP)和骨密度值[腰椎 L1-4及股骨粗隆、左侧股骨颈、Ward 三角区]的水平。结果:两组骨密度及骨代谢指标参数基线值比较,差异无统计学意义(P>0.05)。治疗后3年后,两组BALP 及腰椎 L1-4及股骨粗隆、左侧股骨颈、Ward 三角区BMD 水平显著升高,而CTX和TRAP水平显著降低(P<0.05);而治疗组治疗后 BALP、s-CTX、TRAPL及腰椎 L1-4及股骨粗隆、左侧股骨颈、Ward 三角区BMD水平改变显著优于对照组(P<0.05)。结论:长期培哚普利对高血压合并骨质疏松患者的骨密度,改变骨代谢有一定的疗效。

      Objective: To explore the effects of perindopril on bone mineral density and bone metabolism in middle-aged and elderly patients with hypertension for 3 years. Methods: 138 patients with hypertension and osteoporosis were randomly divided into treatment group (n = 69) and control group (n = 69). Patients in the treatment group were given perindopril and the control group was given levofloxacin benzenesulfonatealone. Treatment for 36 months, the levels of serum type I collagen cross-linked C-terminal peptide (CTX), tartrate Acid Phosphatase (TRAP) and bone specificity of alkaline phosphatase (BALP) and bone density [lumbar L1-4 and femur trochanter, left femoral neck, Ward triangle]were measured before and 36 months after treatment. Results: There was no significant difference between the two groups in bone mineral density, bone metabolic indexs(P>0.05). After 36 months treatment, the levels of BLAP and the BMD of lumbar L1-4 and femur trochanter, left femoral neck, Ward triangle were significantly increased, and the levels of CTX and TRAP were significantly decreased (P<0.05), while after treatment BLAP, CTX and TRAP and the BMD of lumbar L1-4 and femur trochanter, left femoral neck, Ward triangle were significantly better than those in the control group (P<0.05). Conclusion: Long-term perindopril has a certain effect on bone mineral density and bone metabolism in patients with hypertension and osteoporosis.
