      目的探讨体质指数(BMI)、腹部脂肪、臀部脂肪对不同年龄正常人群骨密度的影响。方法 采用双能X线BMD仪和医用身高体重测量仪测定393名正常体检人群腰椎松质骨密度、腹部脂肪含 量、臀部脂肪含量、全身其他部位脂肪含量和身高、体重J要围、臀围,并计算BMI和腰臀比。然后将 人群按不同年龄分为青年组、中年组和老年组,分析骨密度与不同部位脂肪含量的关系。结果(1 ) 体内脂肪含量随年龄增加而增加,女性比男性的变化趋势更明显(P <0. 05 );( 2 )在中年患者中,BMI 与骨密度成正相关(男性组则=0. 237 ’P = 0.018 ;女性组则则=0. 279 ’P = 0. 024 );( 3 )老年男性患者腹部 脂肪含量和臀部脂肪含量与骨密度成正相关(P <0.05 )。而老年女性患者腹部脂肪含量与骨密度成 负相关(则=0.263 ’ P <0.05 )。结论脂肪分布对骨密度的影响是复杂的,其中腹部脂肪和臀部脂肪 可能是影响老年男性骨密度的主要因素,腹部脂肪是中老年女性骨密度的重要负调节因素。因此对 中老年患者需注意减肥方式,以避免骨量的丢失和骨质疏松的形成。

      Objective The aim of this study is to investigaLe the effects of body mass index ( BMI ), abd^iinal fat and hip fat on bon^ mineral density( BMD ) in nomial people of differenL ages. Methods A LoLal of 393 subjecLs were sLudied in Lhis sLudy conducLed from 2010 Lo 2011. The subjecLs were divided inLo youLh groups,middle-aged and elderly groups according Lo Lheir ages. We measured Lhe heighL,weighL, waisL circumference and hip circumference,and Lhen caldiaLed BMI and WHR. BMD, abdominal faL and hip faL were measured by dual-energy X-ray. The relationship of bone mineral density and fat distribution were analyzed. Results 1, Fat mass increased with age, and the trends in women were more obvious than in men ( P < 0. 05 ) ; 2, In the middle-aged paLienLs, BM I was posiLively correlated wiLh BM D ( male :则= 0. 237,P =0. 018 ; female :则=0. 279, P =0. 024 ) ; 3,In old male subjecLs group,abdominal faL and hip faL were posiLively correlaLed wiLh bone mineral densiLy ( P < 0. 05 ). As for Lhe aged female,abdominal faL mass was negaLively correlaLed wiLh BMD (则=0. 263,P < 0. 05 ). Conclusion The effecL of faL disLribuLion on bone mineral densiLy in normal subjecLs is perplexing Lose weighL should noL be overly emphasized in middle-aged paLienLs or elderly people because iL may led lo bone mass loss and osLeoporosis formaLion.
